Out of time outs

Is all this correct? Team A is out of time outs and the coach wants to see about rectifying what he believes is an error. He can still go to the table and request a time out and it will be granted. If he's incorrect or the error cannot be rectified then there will be a technical foul charged for requesting an excessive time out. It will go against the team and not the coach. Are there any case book plays which spell this one out directly? Thanks.

Yes, that's correct.

No, I don't recall any such case plays.

Rule book is wrong, then. 5-12 references "time-outs".
