H12 dunks in warm-ups and is appropriately penalized. H12 is a listed starter and HC wants to remove him from game ASAP.

We allow HC to sub for H12 after the 2 free throws are shot (which BTW were shot by a non-starter from visitors).

Haven't looked it up yet. Correct in allowing H12 to be subbed for after free throws and before throw-in?

Yup. It can only be penalized if you change a starter before the ball became live to start the game.
