Intentional/Flagrant Foul
 
When an offensive player is in the act of shooting (or taking a layup) and he is intentionally fouled. How many shots are awarded?

On a two-point attempt, whether the shot is successful or not, and a successful three-point attempt the shooter is awarded two shots.

On an unsuccessful three-point attempt, the shooter is awarded three shots.

The player was hurt and could not finish the game. The sub shot two free throws and then they shot another 2 free throws and got the ball as well. Does that make any sense?

Not if there was only one intentional foul called.

If there was more than one foul called (say and intentional followed by a T, or a common foul followed by an intentional technical) then there could be 4 free throw attempts.

Not really....sounds like maybe someone was penalized twice for the same offense. From what you describe, the most the free throw shooter should have shot would have been two shots.
