Cancelling the Goal

Reading the Case Book this morning I came across this:

9.13.1 SITUATION: The ball has been released on a field goal try or tap by A1 towards A's basket: (a) A2 ... excessively swings arms or elbows without contacting an opponent. RULING: In (a), the official should sound the whistle immediately for a violation. The ball is dead, the goal is not scored...

Got me thinking, what other situations would A1's try in flight be immediately dead and a goal not count? I came up with:

Violations:
1. Of course, A2 swinging elbows/arms
2. A2 leaving the floor for unauthorized reason

Fouls:
1. A1 player control

That's all I've got. Missing any?

Offensive BI/goaltending.

A free throw violation by the throwing team.

Throw-in by A1 goes through the basket. I don’t believe this is really considered a try, though.

A1 fouled while shooting at wrong basket and it goes in. Again, not actually a try for goal.

Not very likely (except in Padgett's games), and you might argue that the try is no longer in flight, but:

A2 kicks the ball.

A2 standing (or jumping from) OOB or BC touches the ball.

A2 hits the ball with a fist.

Possible:

The lights go out and you're unsure whether the try was good.
