This happened in a well-publicized NCAA D1 men's game last season. I can't remember the game, but we talked about it here. The coach actually told the player to do it so that the clock wouldn't start if the free throw was missed.

UK GA - Goaltended FT
1.2 seconds to go in OT, team B down two, 1 FT remaining for team A. Team B out of timeouts. Coach B instructs player B5 to GT the final FT to prevent the clock from starting in case of a miss... Coach B thought he was choosing to give up 1 point in order to have 1.2 seconds on the clock....but didn't know that it was also a T.
