Incident Report form
 
Hi All,

Our organization is looking to create an incident report form to be used by officials to report any serious incidents in games including ejections of Coaches, fans, fan behavior etc.

At present we have officials calling the President and assignor within 12 hours of the incident but would like a form developed to assist the officials in collecting their thoughts on the situation that occured.

I was wondering if other organizations have a similar form that I can use as a starting point.

The OHSAA provides every official a 4 part form that an official needs to use if a coach or player is ejected. One copy goes to the OHSAA. One goes to the prinical of the school involved. One goes to the Athletic Director. The fourth one is retained by the Official. We have 7 days to file the report after an incident. Failure to do so results in a $50 fine.

First incident results in a 2 game suspension.
Second incident in the same season results in a 1 year suspension.

I have an online form you can use cheap
 
I developed an online form to use that is very similar to the standard incident form. When someone fills one out, it sends an email to an administrator for follow up. The admin comes to a secure area, logs in, and can see the report form. No one else can see them. If you are interested, contact me at [email protected]

just so Ignats does not incur the fine:

OHSAA requires the official to call the school and talk to principal or AD the day following the incident.

OHSAA requires the form to be filed with the school and OHSAA with 48 hours.
