It's noted in the rule book because it's possible according to live-ball/dead-ball definitions.

Consider: An airborne shooter gets fouled on the way up by B1. B2, seeing that the shooter still might make the shot, hits the shooter's arm.
Most of us will pick one, but the rules allow for us to nail them both with the foul.
