Player A goes up for a shot, releases the ball and then makes contact with stationary B player who takes the charge. How many officials give the hand behind the head signal in this case? And when did this change?

The calling official should be the one who's primary area the foul occured in. If more than 1 official is giving the signal then somebody wasn't paying attention to their area.

That's the signal in NFHS, but not in NCAA (it is the PC foul signal in NCAAM, but the play you described is not a PC foul in NCAAM).

For NCAAW team control fouls are signaled with a punch. I am guessing that this is the change you have seen. We have used this signal for at least the last 2 years.
