Is it a rule that the defense team must have someone in the bottom spot for a free throw attempt by the opposing team. I'm talking about NFHS here. I've heard different interpretations, but haven't been able to find it in the book.

That's one time it's not required. There's (at least) a couple more, and more common, examples.

any free throw which will remain dead after the last is missed, example tech, intentional foul.

Also (examples of ball remaining dead) FTs after time has expired for a period.
