shot is released, on weak side offensive player jumps on the back of defense player as the ball hits the rim. Ball eventaully goes into the basket. Do we have a made basket, foul on offensive player, and if in one-and-one we go to the other end to shoot FT's?

If you judge it to be a foul, yes.

probably would not call a foul unless say the offensive player's contact causes defensive player to fly forward or fall and hit the court. Could you ever see yourself calling a foul in this scenario?

Yes. But most of the time I do it, I then (silently) curse myself for not having a more patient whistle.
