Could someone please send me a copy of an evaluation form they use, we need to change ours and we only have 2 upper level officals and they seem to be to busy I'll take any and all Thanks for the help Big Al

I have one that I made up using MS Excel. It's more of an Observation Sheet as we try to stay away from the word evaluate. Email me at [email protected] and I'll send it to you.
