			JR...  From the way I read it I think he is looking for how to do the book because it says how to keep score...  I may be wrong though, happens a lot. If so, there is usually a set of instructions inside the front cover of good score books and the first 2 pages are filled out as an example. Can't say I have seen anything online though.

			That site has great instructions on how to keep the book and the clock, i keep a copy of them in my bag at all times, and if a table official is unsure of his/her duties i'll give it to them to look over.  I find it really helps for them to have written instructions to make reference to during the game if they are unsure of anything.
