Free throws & Ejection

Team A Player is fouled going in for a layup and goes to the ground. Team B player stands over him after the perceived "block" warranting a technical. Player A Stands up and throws a punch warranting a technical and ejection.

What is the correct outcome of this play? Team B (Defense) player gets common foul and T. Team A player gets T and ejected. The T's offset so we should just have 2 free throws. Team A gets 1 team foul, B 2 team fouls.

Who shoots? Who gets the ball?

Assuming FED:

Since the taunt caused the fight, B is also charged with fighting. So, both A and B are ejected.

The fighting fouls offset, so A's substitute shoots the FTs for the common foul, and play resumes as normal.

I hate to nit pick but B1's PF in the play described was not a CF. It was a PF committed against a player in the Act of Shooting.
