Free Throw violations
 
NFHS: Three free throw situations.

How would we administer a violation by the defensive player in a marked lane space, followed by an air ball by the shooter?

How would we administer a violation by the defensive player in a marked lane space, followed by a violation by the shooter (crosses the FT line before the ball hits the rim)

How would we administer a violation by the defensive player in a marked lane space, followed by a violation by a teammate of the shooter not in a marked lane space (crosses the three point line before the ball hits the rim)

All the same way: Simultaneous violations, wave off the shot and go to the arrow to determine who gets possession.

Administer any remaining FTs of a multiple throw situation OR use the AP arrow if there are no more FTs required.
Also consider disconcertion on the FT airball situation.

The only time you ignore the second violation is when both are by plyers in marked lane spaces.

1. Most likely going to call disconcertion here and simply give the shooter another shot. Not a guarantee, but I lean that way on this play.

2. Simultaneous violations. The shot is cancelled, and we move on either to the next shot (if applicable) or the arrow.

3. Same as #2.
