Why four?

The penalty is a DIRECT to the player, INDIRECT to the coach, and A team foul. The INDIRECT is part of the initial act and not a separate act to count towards the team foul.

It's important to understand what an indirect technical foul is.
It is always secondary, something applied to the coach as a consequence for someone under his control getting stupid. It is never its own foul.
