LL Baseball Major Division. Pitcher of record is removed for another pitcher who is discovered to be ineligible because of pitch count after warm-ups but before he throws a pitch to a batter. My question is can the original pitcher be re-inserted. I know when Andy Conyer was UIC the answer would be no. Does anyone know what the current LL opinion is regarding this?

The pertinent language is in Regulation VI:

(b) A pitcher once removed from the mound cannot return as pitcher. Intermediate (50-70) Division, Junior, Senior, and Big League Divisions only: A pitcher remaining in the game, but moving to a different position, can return as a pitcher anytime in the remainder of the game, but only once per game.

The original pitcher is done. He was removed from the mound by another player, despite the fact that that player is not eligible to pitch. Time to find a third player who is eligible.
