I have read some previous postings on this, but I'm still unclear.

Can the batter only advance to first on a missed third strike when the bases are empty?

I read one posting that stated if the bases are loaded, it provided an opportunity for the catcher to intentionally drop the third strike in order to touch the plate and throw to first for an easy double play. (ie:infield fly rule)
Does the number of outs matter?

The batter can attempt to reach first when EITHER (or both) (a) first is unoccupied, OR (b) two are out.

The rule is to allow the batter to run when the catcher can't create an easy double play by "forcing" the batter to run.

So, if the bases are loaded with 2 outs, the batter and baserunners may attempt to advance?

Yup. F2 should just touch homeplate for the force on R3.
