On a pick off to second does the pitcher have to first step back off the rubber with his pivot foot and then turn and throw? Is it legal if as he is lowering his non-pivot foot he continues it over the back of the rubber towards second and makes a throw?

1) No.

2) As long as the motion is continuous, it's legal.
