Written Warning...
 
So in reference to any rule that states "...the coach shall receive a written warning and be restricted to the bench...". Is there a template of paper that umpire should use to write the written warning? do you LITERALLY give the coach a warning ticket (like a cop would do) and tell him he is restricted?

I guess so far i have only seen verbal and ejections.

In NFHS, we tell the coach "That's a written warning", and then note it on our lineup card. No "traffic ticket" is given to the offending coach.

That's a practical use of the lineup card. Use it for noting warnings and ejections as well as defensive and offensive trips. Note the inning, score, who the warning/ejection was given to, what it was for, and what (if any) response was given. Simple notes like that can make writing reports after the game (if needed) a LOT easier.
