During the 1978-79 season, the NBA added a third referee on an experimental basis. The following season, 1979-80, the NBA did not retain the three-referee experiment. In 1988-89, the NBA decided to permanently add a third referee for all games.

I believe the guy on the left is Garretson, before his hair grayed.

The middle guy is definitely Jack Madden.

I think the guy on the right is too tall to be Capers Sr. I'm thinking Hugh Evans.

As of now, I'm going with Darrel Garreston and Madden. Not convinced on the shaft-do (third) guy. Here are pictures of Garretson, Hugh Evans and Capers Jr.
