There are many ways to get subs in if a team really wants to. In a regular game, I'll not be stopping the game to do so without a timeout, violation, or infraction. If it is that not close and near the end, why didn't the coach get them in earlier. The team could even throw the ball OOB on purpose to create a substitution opportunity.
