Do the defensive players have to fill all four slots on a free throw shot?

No they do not.

Defensive players MUST occupy the bottom two spaces. The second 2 spaces MAY be occupied by offensive players. The third marked lane spaces MAY be occupied by defensive players.

This is rule 8.1.4 if you're interested in reading it.

And, the fourth marked spaces are to be left empty.

They also aren't even considered marked spaces.

Also, the second 2 spaces MAY be occupied by the defensive players if the offensive player do not chose to occupy them. And, the third marked spaces MAY be occupied by teh offensive players if the defensive players do not chose t occupy them. In any account, the maximum number of defenders allow along the lane is 4 and the maximum number of offensive players is 2.
